Confusing info

What happened

I was visiting a Specialist for my Blood clot. Aetna received the referral, but i still received a high bill. Talked to them later and they sent me a bunch of confusing paperwork? After reading through the paperwork and 2 phone calls, Im still confused and not understanding why they sent me a bill.
